** IMPORTANT NOTICE **

In addition to the electronic submission, a paper copy of the final report with instructor/volunteer information, including signature, is required to be completed and kept on file by your organization. You must also keep all receipts on file pertaining to this grant. If your organization is subject to federal and/or state audit, and these items are not on file, your organization could owe a refund of the grant monies awarded.



Additional Information and Requirements:

  • Completing the paper copy (PDF) before you start the electronic process may make your job easier!
  • If you start the online portion of the final report and will be away from your computer for an extended time, you must select “Save my progress and resume later” or your data may be lost.
  • An Equipment Inventory Sheet is required to be submitted via email to the coordinator that approved your grant, for any equipment purchased with grant funds.
    • The Equipment Inventory Sheet is located in the paper copy (PDF) of the Outdoor Education Grant Final Report.
  • Instructor/volunteer time reported should be reported to the quarter hour.
    • For example, 15 minutes=.25, 30 minutes=.50, 45 minutes=.75.
    • If an instructor/volunteer reported a time other than to the quarter hour, please round down to the nearest quarter hour.
  • Only submit 1 final report per proposal/event.
  • The grant ID# can be found on your grant approval correspondence.
  • Confirm that all instructors/volunteers have signed their paper copy hours summary.
  • Confirm that all instructors/volunteers have an "activity" listed on their hours summary.
    • Do not list the club name or event name for the activity – it must be the actual PHYSICAL activity that the instructor/volunteer performed.
  • The hours summary sheet should be completed daily - date spans are not acceptable.
  • Submitting a correct and complete final report is essential for federal aid reimbursement that provides continued funding for this grant program, along with your eligibility to apply for future grants.
